Our commitment
Verrais governed by Ontario’s Personal Health Information Protection Act (PHIPA, 2004). We are health information custodians as defined under the Act, and we hold ourselves to the standards that designation requires.
What we collect
We collect personal health information that is reasonably necessary for your care. This typically includes contact details, medical history, current medications and supplements, prior treatment notes, insurance information when relevant, and the clinical findings recorded during your visits.
How we use it
Your information is used to provide care, to maintain accurate clinical records, to communicate with you about appointments, and to direct-bill insurance with your consent. We do not use patient information for marketing. We do not sell or rent any patient information under any circumstances.
Sharing within the practice
Our practitioners may discuss your care with one another when both clinical reason and your written consent allow. Internal discussion is limited to the people directly involved in your treatment.
External sharing
We share clinical information outside the practice only with your written consent, or where compelled by law (for example, certain reportable conditions or court order). When you request a record release to another practitioner, we send the agreed information by secure means and confirm receipt.
How we store it
Records are kept on encrypted electronic systems hosted within Canada and on locked physical storage where paper records exist. Access is restricted to authorised practice staff. We retain clinical records for the periods required by the regulatory colleges that license our practitioners.
Your rights
You may request access to your records, request a correction, withdraw consent, or ask questions about how your information is being handled. Contact us at hello@verrawellness.example. We will respond within thirty days.
